An initial interview is conducted by the Leasing Director who determines basic services needed. For those who require personal care assistance, the Wellness Director conducts a personal assessment to determine care needs and ability to meet assisted living qualifications.
The Wellness Director, a licensed practical nurse, reviews a physician’s medical report completed by the prospect’s personal physician. The director uses a standard assessment form, addressing all activities of daily living — including bathing, dressing, grooming, toileting, meals, etc. — as well as a mental evaluation to determine dementia level, if any.
A detailed plan of care is developed at the time of the assessment with the resident, family and the review of the physician’s written report. It is reviewed one month after move-in and every three months thereafter unless there is a change in the resident’s condition such as a hospitalization.
Vernon Woods staff members provide assistance with daily life activities such as dressing, bathing, toileting, incontinence, transfers from bed to wheelchair, and other mobility issues.
Residents may manage their own medications provided Vernon Woods has received physician approval and written authorization.
The state of Georgia does not allow for nursing care services to be provided by Vernon Woods. Residents who need continuous nursing services will be referred to a skilled nursing facility that can better meet their needs, such as Florence Hand Home or Twin Fountains Home of WellStar West Georgia Medical Center. Temporary nursing services ordered by physicians may be contracted by the resident with an outside agency. The resident is responsible for these charges, and these services must be pre-approved by Vernon Woods.
