How MyPortal Works for You

Those looking to self enroll can click here. To self-enroll, you must use your medical record number.You may find your medical record number on your patient armband or paperwork. It is the nine-digit number following M (Example – M000987654). If you don’t know your medical record number, you may call the Medical Records Department at (706) 845-3958, from 7 a.m. to midnight on weekdays and from 8 a.m. to 4:30 p.m. on Saturdays and Sundays, and they will provide the number to you.
