Portal Self Enrollment

Enroll Yourself in Patient Portal

To Self-Enroll: 

  • Click on the MyPortal link at the top of your page.
  • Complete the enrollment form. Your Medical Record Number is required, and your current email address must be on file in Medical records.
  • A one-time use ID, password and link to the Portal will be emailed to you.
  • Click on the link in the email to access the Portal.
  • Enter your one-time use ID, password and security question and click Log On.
  • Enter your new log-on user ID and password.
  • Explore the Portal!

For assistance with questions regarding MyPortal, call the Medical Records Department at (706) 845-3958 from 7 a.m. to midnight on weekdays and from 8 a.m. to 4:30 p.m. on Saturdays and Sundays. You may also visit the Contact Us section on the MyPortal homepage.

To self-enroll, you must use your medical record number. You may find your medical record number on your patient armband or paperwork. It is the nine-digit number following M (Example – M000987654). If you don’t know your medical record number, you may call the Medical Records Department at (706) 845-3958, from 7 a.m. to midnight on weekdays and from 8 a.m. to 4:30 p.m. on Saturdays and Sundays, and they will provide the number to you.
