Payments and Insurance
Full payment is expected at the time of service. Our office participates with several PPO plans. Since all plans are different, please bring your insurance card with you at the time of your office visit. If your insurance has a co-pay, you are asked to make that payment at the time of service. You are responsible for any deductibles and co-insurance. Please contact your insurance company for details regarding your insurance plan. As a courtesy to our patients, we will file all primary, secondary and tertiary claims.
Our doctors are Medicare participating physicians. Medicare patients are responsible for their yearly deductible beginning January 1 of each year and for 20% of the Medicare allowable. We file all Medicare supplemental claims for our patients.
Out patient procedures may be scheduled at Southern Surgery Center or WellStar West Georgia Medical Center. Patients may chose which facility they prefer.

Red Flag Policy
The purpose of this Red Flag Policy is to establish procedures to follow to protect patient's medical and financial information in compliance with the Red Flags Rule issued pursuant to the Fair and Accurate Credit Transactions Act (FACTA) of 2003
NEW PATIENT - Must provide photo ID to receive services.
ESTABLISHED PATIENT - Must provide two other types of ID, such as social security card or credit card, and be able to verify social security number, date of birth, address and telephone number.
Photo ID must be on file to receive services.
CHANGE OF ADDRESS - Patient is required to provide updated driver's license or another document with the patient's name and updated address, such as a current utility bill.
These new changes are being implemented to protect patient from Medical Identity Theft.
